Fall 2015 Registration Instructions

  1. MAC users MUST use Internet Explorer, Netscape or Firefox

  2. Read Welcome letter from our Regional Commissioner by clicking WELCOME

  3. Read the Volunteer Position Descriptions by clicking DESCRIPTION. Decide which position would be best for you. YOU MUST CHOOSE ONE PER FAMILY

  4. Read the Region 69 Registration Agreement by clicking AGREEMENT. You agree to this when you e-sign your Player Registration Form on www.eAYSO.org. (Read Kid Zone Pledge)

  5. Returning players may continue to register however waitlisting is possible after April 15th. The PRIORITY DEADLINE for Submitting RETURNING Player Applications and payment was April 15th.  

  6. NEW PLAYER REGISTRATION must be completed with an online AND on-site registration.  If the player is 6 years old or older on July 31st, 2015 an evaluation will also be required.   New player registration will be at Paul Revere Middle School on April 25, May 17 and June 7 from 3-5pm.

  7. You MUST FIRST register online and then bring eAYSO Player Registration Form, at least one eAYSO Volunteer Form, proof of online payment (or check), and proof of age to the Registration/Evaluation.  DO NOT MAIL FORMS.

  8. The Registration Fee is $285. PAYMENT may be made online by credit card on www.eAYSO.org, or by check with player name clearly indicated.  

  9. Registration involves 1 ONLINE step ONLY for Players AND Volunteers.   (Site is down most Monday evenings 6pm-Midnight)

eAYSO Instructions for RETURNING Families  - Read instructions then click on www.eayso.org (New window opens)

  1. Returning Families:  Your login is the e-mail address you used last season. Please DO NOT set up a new account. 

    You MUST update and electronically sign and submit all forms, for both players and volunteers—EACH YEAR

  2. Click on Apply as a NEW Adult Volunteer OR Apply as a RETURNING Adult Volunteer. At least one parent from each family must submit a Volunteer Form online even if "OTHER". If you want to COACH, ASST COACH, or REFEREE, you should so indicate on this form.    Look in the right column for youth volunteer forms for your children if they are volunteering to ref, coach or to be a VIP buddy.  

  3. * new Adult Volunteer ONLY:  Click on Print Form and print 1 copy of the form. Mail this form and a copy of your driver’s license to the office ASAP. 150 S. Barrington Ave, #2, Los Angeles, CA  90049.

  4. Click on APPLICATION for a returning player, or new (sibling) player,  in the left column.

  5. Verify or enter your child’s birthday and other information.  Do this for each child applying.

  6. Make sure BOTH parents have been entered with their CURRENT email addresses.

  7. PARENTS JOBS - At least one parent must volunteer for a position even if "OTHER".  If you want to COACH, ASST COACH, or REFEREE, you should so indicate.   

  8. For payment, you may send a check, or you may use the credit card option.

  9. Returning players may continue to register however waitlisting is possible after April 15th.

  10. U16 & U19 PLAYERS ONLY:  Copy of Passport or Birth Certificate and 2 one inch COLOR photos REQUIRED. Mail this to the office ASAP.

AYSO Youth Volunteers

  1. If you did not previously perform the registration process above, you can do this by going to eAYSO, log in with family login and click on My eAYSO.

  2. Proceed with this step if your child is volunteering for youth referee, coach, VIP buddy or other positions.

  3. Click on the appropriate Youth Volunteer application (i.e. New or Returning) in the right column
